Proposed classification scheme for MPGN into immune complex–mediated and complement-mediated MPGN, based on immunofluorescence microscopy (IF) findings. Immune complex–mediated MPGN is characterized by the presence of immunoglobulins (Ig) and/or C3, while complement-mediated MPGN is characterized by the presence of bright C3 and absent/scant Ig. Evaluation of immune complex–mediated MPGN should include work-up for monoclonal gammopathy, autoimmune diseases, and infections. On the other hand, evaluation of complement-mediated MPGN should include work-up for functional, acquired, and inherited abnormalities of the alternative pathway of complement.
